Order repeat prescriptions in writing, by post, or by calling in at the Surgery.
You can also request your repeat prescription on-line. Download and return this form to register with My Health Online, this one for children / carers. Welsh versions are also available for adults and children / carers.
Requests are no longer accepted by telephone. You are advised to allow 2 working days before collection.
All results of investigations undertaken in the surgery may be obtained from 13:30 onwards.
Any investigations done at the hospital are usually given at your next hospital appointment.
