Order repeat prescriptions in writing, by post, or by calling in at the Surgery.

You can also request your repeat prescription on-line. Download and return this form to register with My Health Online, this one for children / carers. Welsh versions are also available for adults and children / carers.

Requests are no longer accepted by telephone. You are advised to allow 2 working days before collection.


All results of investigations undertaken in the surgery may be obtained from 13:30 onwards.

Any investigations done at the hospital are usually given at your next hospital appointment.

Out of Hours Service

To contact a Doctor outside of normal surgery hours telephone 111.

111 is the NHS non-emergency number. They will ask you a series of questions to assess your symptoms and immediately direct you to the best medical care for you.
NHS 111 is available 24 hours a day, 365 days a year. Calls are free from landlines and mobile phones.

Hywel Dda University Health Board – Protecting our Community from Coronavirus

In view of the current outbreak we have had to make changes to our appointment system.
As from Monday 16 th March, we will not be offering pre-bookable appointments with a GP
or Nurse. All appointments will be bookable on the day by telephoning the surgery.

Please do not present at reception to request an appointment.
